1. The Program
Effectiv Inc. ("Effectiv," "we," "us," or "our") sends text messages on behalf of healthcare providers that use the Effectiv platform (the "Program"). Messages include links to health check-ins, reminders to complete them, and other messages related to your care program with your provider. We do not send marketing messages.
2. How You Opt In
You agree to receive text messages when you give your healthcare provider permission to send you check-ins by text, or when you agree to text messages in an Effectiv check-in. Consent to receive text messages is not a condition of any purchase.
3. Message Frequency
Message frequency varies with your care program's check-in schedule and reminders.
4. Costs
Message and data rates may apply. Contact your wireless carrier for details about your plan. Effectiv does not charge you for text messages.
5. How to Opt Out
Reply STOP to any message to stop receiving text messages. You can also reply STOPALL, UNSUBSCRIBE, CANCEL, END, or QUIT. You will receive one message confirming that you have opted out, and no further messages after that. To start receiving messages again, reply START.
Opting out of text messages does not end your care with your provider. Your provider may still contact you in other ways.
6. Help
Reply HELP for help, or email hello@effectivhq.com. Replies to our messages are not read by a person, so please do not text questions about your health. Contact your healthcare provider directly. If you have a medical emergency, call 911.
7. Carriers
Wireless carriers are not liable for delayed or undelivered messages. Message delivery depends on your carrier and is not guaranteed.
8. Privacy
We use your mobile number only to send messages for the Program. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. See our Privacy Policy for more about how we protect your information.
9. Changes
We may update these SMS Terms from time to time. Changes take effect when posted on this page.
